If your health insurance plan requires a referral to see a specialist, please bring the referral with you at the time of your office visit or procedure. If you fail to bring the referral and are unable to have your primary care physician's office fax it to our office, we will have to reschedule your appointment.

If you do not have health insurance, we still welcome you as a patient.

Billing for endoscopic procedures are billed seperately from the physician services. Therefore, you will receive a bill for use of the facility (Endoscopy center) and from our office for the physician's services.

Also, if any biopsies are obtained during your procedure, you may receive another bill from Caris Diagnostics. Our business office does not process these bills but we will be happy to provide you with contact information for Caris Diagnostics.
